Parents of incoming Kindergarten, Second & Sixth Grade Students,

 

The State of Illinois requires all students in grades K, 2 & 6 to submit a completed oral health exam (dental) form to the school by May 15th of those school years.

Please note: The rules allow for the exam to have been performed up to 18 months prior to the May 15th deadline.

​

If you cannot get an appointment for a dental exam by May 15th, you are allowed an additional 60 days-through July 15th to get your child's dental exam done as long as you submit proof of an upcoming appointment.
