Sports Physical

ALL 3 FORMS MUST BE COMPLETED
to ensure your athlete and family are in compliance with their participation. All have virtual options to provide you with easier access and organized paperwork for us. Due to league play, we will need to have an updated sports physical.

The only ones that will NOT need a sports physical will be our non-league teams.

If you are interested in coaching, please contact the Athletic Director. You must be Virtus trained (Information will be provided on how to get certified), take an online concussion test, and complete a CANTS form.
